Support from a 'social prescribing link worker'.
“We want to support you in a way that works best for you, so we can offer virtual and face to face appointments”
What is Prescription for Wellbeing?
“A prescription for wellbeing gives you the support from a social prescribing link worker, who will signpost you to a range of free community groups, activities and services that will boost your wellbeing and help you to feel happier”.
At the first appointment, your Link Worker will work closely with you on a one-to-one basis. They will listen to you, support you to make plans and help you choose what will work for you to improve your wellbeing, keeping you in control. You will receive ongoing support from your Link Worker whilst accessing the service.
We can provide support with:
- Money worries
- Meeting new people
- Improving your lifestyle
- Keeping your home warm
- Getting involved with community groups
- Learning new skills or getting into work
How do I get support?
